Assessment forms
Prior to booking an appointment, there is an option to complete a short assessment form. This asks for details which include your name, age, occupation, email and phone number. A brief outline of the health complaint is also requested. However, completion of the Assessment Form is optional. Any information which is provided will form part of the initial consultation and will be safely stored & retained for at least seven years. However, under GDPR you have the right to ask for data to be deleted/erased. None of the information will be used for marketing purposes. Who we share your data with
Your consultation information is completely confidential and will not be shared with anyone else unless legally required to do so.
How long we retain your data
Information that is provided as part of your consultation will be safely stored and retained for at least seven years. However, under GDPR you have the right to ask for data to be deleted/erased.
If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognize and approve any follow-up comments automatically instead of holding them in a moderation queue. For users that register on our website (if any), we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.
What rights you have over your data
If you have an account on this site, or have left comments, you can request to receive an exported file of the personal data we hold about you, including any data you have provided to us. Under GDPR you can also request that we erase/delete any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes.
